What EXACTLY shows up on a high school transcript? SCARED OUT OF MY MIND!?

I'm only a freshman in high school, but 2nd quarter; I didn't do so great. I take Honors Living Enviornment, and got a 75 on my midterm. That's bad enough...but it didn't stop there. Please don't laugh, but I got a 57 on my Geometry Honors midterm. My average for 2nd quarter was an 80. I had a really bad day on midterm day, the night before, I found out that my grandfather who I am very close to was diagnosed with lung cancer. DOES THIS SHOW UP ON MY HIGH SCHOOL TRANSCRIPT WHEN I APPLY TO COLLEGE? I'm so scared. I really want to become a dentist; I've been working really hard, and I want to go to UPENN. This quarter, I've recieved 98s on all my Geometry Honors tests. It was just that day. Is it only the final average which shows up on a high school transcript? I'm so scared. Does this ruin my chance of getting into college? UPENN is my dream.
